Propose this issue be closed as the issue raised here
is also raised in issue #139, along with a few other
issues in the same section.

Clause 19 (and also 17) has many cases like that
(e.g., "filename", "level", "time_unit", "time_precision").

These clauses were not formal about syntax, BNF, etc.
Note that with rare exceptions, they also do not appear in Annex A.

However, with respect to the italics, you are correct.
This is a remnant from 1364-1995, in which the italics were used to
indicate that syntactically,
formal_argument_identifier is identical to identifer.

I will file that in a bug report, along with a couple of other issues
I see now.
